When coming in for your first appointment, please bring the following with you:

• Print and fill out the New Patient Information Form found below
• Current medication list
• Insurance Card
• Photo ID (such as a driver's license)
• Insurance CoPay if applicable

Please arrive 15 minutes prior for appointments.   If our nurse has not gone over your Health History with you, please arrive 30 minutes prior to your first appointment time.
